What is a good way to format/structure a semantic differential questionnaire?

For a project I have adopted a prototyping methodology and I will be gathering user feedback during each iteration for a mobile app - the feedback will then be used to improve the app. A majority of the feedback will be gathered in the form of questions using semantic differential.

However, I am a little confused as to how I should format and structure my questionnaire. At the moment, I have questions like this:

But having done some research, this seems to be the way to do it:

I think it may also be difficult to represent data gathered using the style shown in the first image - it's feels more like a 'Likert scale'.
