Third-Party plugin for organising questions on database [closed]

I am looking advice on a third party plugin that works with PHP 7 to organise data related to questionnaires that can have sub-questionnaires by also providing the front-end UI for a questionnaire and back-end functionality that allows categorisation of the data.
