How to design ‘Send Alert Mail’ Screen [on hold]

I have screen that configuring e-mail notifications however design looks a bit weird and it may needs re-design.

It looks a bit overvol/not good.

I want to make it cleaner and didn't decide the where should i put the checkboxes and the notification logs.

I need your ideas to identify the problem and solutions.
