Contextual questions in forum builder

In the product I’m working on there is a part where you can build forms for your employees to fill out. There is a title to the form, contextual questions and variable questions.

Without getting too in-depth, the contextual questions are questions that are created to have answered once by the front line employee and variable are to be asked each time there is a new set. For example, I have a lot of chickens being sent to my factory and in this lot there may be 1000 birds, which require perhaps 50 trucks to arrive with a load of birds. I need to note how many are dead on arrival so in the forum I fill out, the contextual question is the lot number but the variable is how many were dead on arrival - so I will fill out a sheet for each time a truck arrives, however, I won’t re-answer the contextual question “what lot #” because for the next 49 trucks, it will be the same lot - if that makes sense.

The reason for the contextual question is because that is how we link data to create reports. There may be other forums created using lot number x but for how many birds arrived dead and were brown one for dead and one for colour - that would help me generate a report on dead brown chickens. The reason is because it could be a different employee filling out a sheet based on colour and another employee noting the dead on arrival.

The problem is that users are getting caught up on the contextual question portion of the forum builder. I’m thinking it’s the wording. They understand there is a question that their employees answer once so that it links to that specific thing, but also that there are questions to ask around that contextual questions. (government forums for example, you put your name or Social Insurance number once, and if they wanted to they’d be able to pull all the data using your name or SIN because it’s on every form you filled out)

That was long winded but I’m wondering if anyone has any suggestions what we could do to make it more understandable.

The context question is needed for business use because it’s the link between the data and the reports. Example: I want to know a and b about lot 78, in this scenario any sheet filled out would have had a contextual question “what lot number”.

Ps Sheets are what the employees fill out since they can fill out multiple. It’s basically an instance of the form.
