Conditional checkbox

I have situation to handle checkbox event , if user unselect the checkbox than underneath dropdown control give option to select single value from dropdownlist. If user selected the checkbox above than user have to fill max and min value underneath with inputbox.What is best to handle situation combine to single control.see the image attached[![enter image description here][2]][2]
