Best way to select answers for a questionnaire which carries points based on the option selected

I am designing a screen in which a doctor/nurse fills the form based on the patient's symptoms. Each answer carries points which gets added up in the end to calculate the severity of the illness. I have added a screenshot for reference. What's the best way to do this?
